Frequently Asked Questions
The state licenses a nurse registry to refer staff that will provide nursing services to healthcare facilities and private individuals on a contractual basis.
You do not have to be in your home to receive nursing services. We can refer caregivers to provide you with services in a private home, hospital, nursing home, assisted living community, rehabilitation facility, or independent living community.
Rates for caregiver services vary depending on the type of caregiver requested and the level of care needed to meet your specific needs. Actual cost can only be determined after an assessment of your or your loved one’s needs.
We accept long-term care insurance plans that cover home health care services, checks, and credit card payments. We will accept an assignment of benefit for your long-term care insurance policy and bill them directly. Please note that all insurance policies are different, and coverage varies.
A representative from Horizon will assist you in contacting your insurance company to verify your specific coverage if needed.
Please remember that although we will bill your insurance company directly if payment is not received in full for services rendered to you, you are ultimately responsible for payment.

We have been able to provide caregivers to clients in emergencies within a few hours. However, 24–48 hours’ notice is preferable.
If you are dissatisfied with the assigned caregiver for any reason, please contact our office immediately at 561-432-1932 or 866-432-6679 so we may replace the caregiver. Your safety and satisfaction are our top priorities.
