Required Qualifications

To become a part of our registry of caregivers, we require the following information and documentation to ensure that we maintain a high standard of care.

Please provide us with your updated resume, proof of relevant certifications, and references from previous employers or clients.

Additionally, we ask that you complete a background check and participate in an interview to discuss your experiences and approach to caregiving. If you have any questions, please reach out to us.

  • Level 2 criminal background screening
  • Valid Florida driver’s license or Florida identification card
  • Social security card or tax identification number
  • Birth certificate, US passport, naturalization certificate, work permit, or green card
  • Current CPR card
  • Current TB test or chest X-ray
  • Physical examination form (completed by a physician within the last 6 months)
  • Verifiable references
  • Proof of automobile insurance (must be current at all times)
  • Annual driving record check

  • Certified nursing assistant license (if applying for CNA position)
  • Home health aide certificate (at least 75 hours of training)
  • Registered nurse license (if applying for RN position)
  • Licensed practical nurse license (if applying for LPN position)
  • Professional liability insurance (malpractice insurance)

  • Understanding of Alzheimer’s care
  • HIV/AIDS training
  • Domestic violence awareness
  • Valid CPR card
  • Assisting with self-administration of medication
  • Medical errors training for CNA, RN, and LPN
  • Caregiver license (issued by Palm Beach County for HHAs and Companions)
